Medication support

The right medicines, at the right time.

From a friendly reminder to administering every dose, our carers help you take your medicines safely at home, and record each one so you and your family can see it's done.

Three levels of support

Everyone's different, so we agree the level of help with you and write it into your care plan. It's reviewed whenever your medicines or needs change.

  • Prompting. A reminder that it's time for your tablets, and a check that you've taken them.
  • Assisting. Help with the practical parts, like opening packs, pouring liquids or putting in eye drops, while you stay in charge of your medicines.
  • Administering. Your carer gives you your medicines as prescribed, when you'd rather not or can't manage them yourself.

Every dose recorded

Carers record each medicine on an electronic medication record as they give it, so nothing is missed or doubled up. If you agree, family members can see these records through our care app. Missed or refused doses are followed up the same day.

Working with your pharmacy and GP

We can collect prescriptions, help arrange repeat orders, and talk to your pharmacist or GP when something changes or doesn't seem right. If your medicines would be easier to manage in a different format, we'll raise it with the pharmacy for you.

What we don't do

Some health tasks, such as injections or dressings for wounds, are usually carried out by district nurses rather than home carers. We'll work alongside your nursing team and make sure everyone knows who is doing what.

Trained and checked

Carers who support with medicines are trained and assessed as competent before they do so, and their practice is checked regularly. Our approach follows NICE guidance on managing medicines for adults receiving social care in the community.

Part of a wider visit

Medication support is usually part of a longer visit that might include breakfast, washing and dressing, or settling in for the evening. Visits start at 30 minutes. See our fees and funding page for prices.
